Authorised Absence of Leave form

Parents should use this form to notify us of any planned term time leave for authorised reasons ie medical appointments, funerals, religious observations, external exams (eg dance, sports, music) and other school visits.   We do not authorise absence during term time for holidays and travel, we expect all holidays to be taken during the school holidays with no overlap.  Please notify our Headteacher Mrs Cook of any intention to do this by emailing admin@chi.rbksch.org, at lease two weeks in advance.

To be completed by parents where their child has additional health needs or a medical condition, we ask you to complete this form fully and accurately so that we are able to support your child in school.

This form should be completed fully if your child needs to be given any medication/medicine during school hours.
